Transition

There can be little doubt that one of the key issues for a young people with a learning disabilities looking forward to their futures with or without the possibilities of employment is how their “transition” is organised.  The term has always referred to their move from children’s services to adult’s services.

This 60 page PDF, Personalised Transition summarises a new way of organising support to disabled young people and their families.  It is written by Alison Cowen with contributions from Simon Duffy and Pippa Murray.

Personalised Transition is a system for enabling young people with complex needs to leave school and achieve active citizenship within their communities. 

Personalised Transition was first developed at Talbot Specialist School in Sheffield in 2006 as part of a collaboration between families, school, Sheffield City Council, NHS Sheffield and the Learning and Skills Council.  The model began to be used in 2007 and is now being further extended for use across the whole city.
